Mirikizumab one-year data show steroid-free disease control in ulcerative colitis and Crohn's disease

One year of mirikizumab (Omvoh) produced steroid-free outcomes in two Lilly trials. In LUCENT-2, 27.7% of ulcerative colitis patients achieved comprehensive steroid-free disease control, and in VIVID-1, 20.4% of Crohn's disease patients achieved steroid-free deep remission. The results extend mirikizumab's reported efficacy profile to composite one-year endpoints in both conditions.

Why it matters: New one-year composite endpoint data for mirikizumab in both ulcerative colitis and Crohn's disease.

Lonvoguran Ziclumeran - In Vivo CRISPR Gene Editing in Hereditary Angioedema.

A single intravenous infusion of the in vivo CRISPR gene-editing therapy lonvoguran ziclumeran cut monthly hereditary angioedema attack rates by 87% relative to placebo in a phase 3 trial of 80 patients with C1 inhibitor deficiency. From weeks 5 through 28, the mean monthly attack rate was 0.26 with lonvo-z versus 2.10 with placebo (P<0.001), with no serious or grade 3 or higher adverse events in the treatment group. The results support a one-time infusion replacing ongoing preventive therapy for this disease.

Why it matters: Phase 3 evidence that one-time in vivo CRISPR editing nearly eliminates attacks in hereditary angioedema.

Personalized neoantigen-targeting cancer vaccines in ovarian cancer: a phase 1 trial.

A personalized neoantigen long-peptide vaccine (NeoVax with poly-ICLC), combined with delayed anti-PD-1 antibody, was feasible and safe in ten patients with platinum-sensitive high-grade epithelial ovarian cancer after platinum doublet chemotherapy. Nine of twelve evaluable Cohort B patients were vaccinated (75%, 90% CI 47-93%), over 70% of targets remained detectable in cell-free DNA despite a median 6.5-month biopsy-to-vaccine interval, and biomarker analysis suggested biologic activity in six of ten patients. Resistant tumor clones showed defects in antigen presentation, consistent with vaccine-driven immune pressure.

Why it matters: Shows personalized neoantigen vaccination is feasible and immunologically active in ovarian cancer, a tumor type with few such data.

Long-Acting HIV Therapy in African Adults with Adherence Challenges.

Long-acting injectable cabotegravir plus rilpivirine every 8 weeks maintained viral suppression in African adults with HIV and adherence challenges, matching daily oral dolutegravir-based therapy. At week 48, 91.0% of the long-acting group (244 of 268) had a viral load below 50 copies per milliliter versus 89.2% of the oral group (240 of 269), meeting noninferiority. Confirmed virologic failure occurred in 5 participants (1.9%) on long-acting therapy and none on oral therapy.

Why it matters: It tests long-acting injectable HIV therapy in the population most affected by adherence failure: African adults with suboptimal adherence.

Phase 2 Trial of Icotrokinra for Patients with Ulcerative Colitis.

Once-daily oral icotrokinra, a peptide that blocks the interleukin-23 receptor, beat placebo for clinical response at week 12 in adults with moderately to severely active ulcerative colitis: 63% at 400 mg, 58% at 200 mg and 55% at 100 mg versus 27% on placebo (P<0.001 for all). The 400-mg group also showed more clinical remission (30% vs. 11%), symptomatic remission (46% vs. 19%) and endoscopic improvement (37% vs. 14%). It offers an oral alternative to injected interleukin-23-targeted antibodies.

Why it matters: An oral interleukin-23 receptor blocker matched injected antibodies' mechanism with positive phase 2 results.

Vanda freezes Novartis-originated social anxiety program after late-stage disappointment

VQW-765, an alpha 7 nicotinic acetylcholine receptor partial agonist licensed from Novartis, failed its Phase 3 trial in social anxiety disorder. In study VP-VQW-765-3201, 546 adults with elevated baseline anxiety received VQW-765 or placebo before a standardized social stress test, and the drug did not significantly reduce distress versus placebo in female participants, the primary outcome. Vanda has put the program on hold pending further analysis.

Why it matters: A Phase 3 failure overturns promising sex-stratified Phase 2 findings and halts a licensed social anxiety program.

VERIFY Phase 3: Rusfertide Improves Hematocrit Control and Cuts Phlebotomy Need in Polycythemia Vera

Rusfertide improved hematocrit control in polycythemia vera patients who were phlebotomy-dependent with poorly controlled hematocrit. In the VERIFY phase 3 study, 77% of rusfertide patients needed no phlebotomy between weeks 20 and 32 versus 33% of controls, and 63% maintained hematocrit below 45% versus 14%, with mean phlebotomies of 0.5 versus 1.8 (all P<.0001). It offers an alternative to repeated phlebotomy in these patients.

Why it matters: Phase 3 data show rusfertide reduced phlebotomy dependence in polycythemia vera across multiple endpoints.

Intermittent or Continuous Renal-Replacement Therapy in Acute Kidney Injury.

Starting renal-replacement therapy with intermittent hemodialysis rather than continuous renal-replacement therapy was not noninferior in critically ill adults with acute kidney injury. The trial stopped early after 554 patients were randomized because of higher mortality with intermittent hemodialysis: day-90 death occurred in 63.1% versus 53.5%, and major adverse kidney events in 78.1% versus 71.6%. Clinicians choosing an initial modality now have mortality evidence favoring continuous therapy.

Why it matters: It gives clinicians comparative mortality evidence for choosing the initial renal-replacement modality in acute kidney injury.

Effect of Vasopressin Initiation Threshold on Septic Shock Mortality: The VASSPR Cluster Crossover Randomized Strategy Trial.

A hospital-level strategy of starting fixed-dose vasopressin at 0.1 µg/kg/min norepinephrine-equivalent did not reduce 28-day mortality compared with starting at 0.4 µg/kg/min in septic shock (31.8% vs 33.0%; adjusted OR 0.98, 95% CI 0.87-1.06). The trial randomized 13 hospitals in a cluster-crossover design and included 3959 patients; kidney replacement therapy-free days were also unchanged. Clinicians can defer second-line vasopressin without a mortality penalty.

Why it matters: Large pragmatic trial showing the timing of adding vasopressin in septic shock does not change mortality.

Isoflurane-Based Inhaled Sedation in Mechanically Ventilated Adults With Acute Hypoxemic Respiratory Failure: The SAVE-ICU Randomized Clinical Trial.

An isoflurane-based inhaled sedation strategy did not reduce hospital mortality in mechanically ventilated adults with acute hypoxemic respiratory failure compared with usual intravenous sedation. In the SAVE-ICU trial of 780 adults across 14 Canadian ICUs, 42.5% died in the inhaled sedation group versus 43.8% with intravenous sedation (risk ratio, 0.98; P = .85). Isoflurane patients received more adjunctive therapies for hypoxemia and had fewer delirium- and coma-free days.

Why it matters: Large randomized trial shows no mortality benefit and some signals of harm for isoflurane sedation in ventilated AHRF patients.

DESTINY-Breast11 RCB Analysis Shows T-DXd-THP Cuts Residual Burden in High-Risk HER2-Positive Early Breast Cancer

In the phase 3 DESTINY-Breast11 trial in high-risk HER2-positive early breast cancer, neoadjuvant T-DXd followed by paclitaxel, trastuzumab and pertuzumab (T-DXd-THP) achieved a pCR rate of 67.3% versus 56.3% with dose-dense doxorubicin-cyclophosphamide followed by THP, and a prespecified RCB analysis presented at ESMO BC 2026 showed an RCB-0+I rate of 81.3% versus 69.1%. The RCB index distribution shifted toward lower residual burden (Q3/Q4 boundary 1.01 vs 1.38, P=0.0059), and among patients not reaching pCR the median RCB was 1.5 versus 1.7. The findings add pathologic-response detail beyond the binary pCR endpoint to guide postoperative adjuvant decisions in an anthracycline-free regimen.

Why it matters: Phase 3 RCB data quantify how T-DXd-THP reduces residual disease beyond pCR in high-risk HER2-positive early breast cancer.
