Long-Acting HIV Therapy in African Adults with Adherence Challenges.

Long-acting injectable cabotegravir plus rilpivirine every 8 weeks maintained viral suppression in African adults with HIV and adherence challenges, matching daily oral dolutegravir-based therapy. At week 48, 91.0% of the long-acting group (244 of 268) had a viral load below 50 copies per milliliter versus 89.2% of the oral group (240 of 269), meeting noninferiority. Confirmed virologic failure occurred in 5 participants (1.9%) on long-acting therapy and none on oral therapy.

Why it matters: It tests long-acting injectable HIV therapy in the population most affected by adherence failure: African adults with suboptimal adherence.

Phase 2 Trial of Icotrokinra for Patients with Ulcerative Colitis.

Once-daily oral icotrokinra, a peptide that blocks the interleukin-23 receptor, beat placebo for clinical response at week 12 in adults with moderately to severely active ulcerative colitis: 63% at 400 mg, 58% at 200 mg and 55% at 100 mg versus 27% on placebo (P<0.001 for all). The 400-mg group also showed more clinical remission (30% vs. 11%), symptomatic remission (46% vs. 19%) and endoscopic improvement (37% vs. 14%). It offers an oral alternative to injected interleukin-23-targeted antibodies.

Why it matters: An oral interleukin-23 receptor blocker matched injected antibodies' mechanism with positive phase 2 results.

J&J’s innovative medicines carried Q3 as medtech disappoints, but newer medicines absent

Johnson & Johnson posted overall Q3 revenue of $25.4 billion, with innovative medicines up about 8% to $16.76 billion while medtech grew only 2.5% to $8.64 billion. Tremfya sales rose 68% to just under $2.4 billion, Darzalex grew 15% to $4.24 billion, and Icotyde, approved in March for plaque psoriasis, has reached about 20,000 patients against a $10 billion sales expectation. Charges of $5.52 billion for the talc settlement and a $339 million impairment cut EPS to about 9 cents, and CFO Joseph Wolk pledged internal investment to fix medtech.

Why it matters: J&J's results open pharma earnings season and detail medtech weakness, drug launch progress and talc settlement charges.

Pharma’s $500B patent problem will come in waves — not as a cliff

Evaluate forecasting data from Norstella show more than $500 billion in worldwide prescription drug sales at risk from generic competition between 2026 and 2032, with the largest single-year exposure of $158 billion in 2031. Merck is most exposed near term as Keytruda nears expiry in 2028, a year with $99 billion in sales at risk, while AbbVie faces roughly $50 billion of exposure in 2032 when Rinvoq and Skyrizi lose protection. Patent-exposed companies have responded with dealmaking since the start of 2024, led by Merck's six deals totalling $30.9 billion and Lilly's $27.2 billion across nine.

Why it matters: Quantifies the multiyear patent-expiry risk facing major drugmakers and the dealmaking it is already driving.

TRex Bio prices $117M IPO to support immune drug work

TRex Bio priced a $117 million IPO, selling 8,333,334 shares at $14 each, and begins trading Friday on Nasdaq under "TRXB." The company is developing Treg-activating drugs for immunological conditions, including TRB-061, in a Phase 1 trial with topline data expected in 2027, and TRB-071 for inflammatory bowel disease, which could enter testing next year. Eli Lilly, TRex's largest equity holder at about 24% pre-IPO, and Johnson & Johnson have licensed or optioned treatments discovered by TRex, which has raised $220 million in venture financing to date.

Why it matters: IPO shows continued investor appetite for immune-focused biotechs, with six of 23 drug developers going public in 2026 working on immune diseases.

Q3 layoffs down YOY, with 90% fewer biopharma employees affected

BioSpace tallies show biopharma layoffs fell sharply year over year in Q3, with a 56% drop in companies making or projecting cuts and a 90% plunge in affected employees; the 27 companies and 2,568 people affected are the lowest quarterly figures since tracking began in 2024. Eight Big Pharma companies accounted for cuts of 1,845 employees in the quarter, including Bristol Myers Squibb letting go of 265 people in Princeton, New Jersey, GSK closing a German vaccine site with about 650 job losses, and Novartis, whose 2026 disclosures total at least 922 people. Novo CEO Mike Maziar Doustdar said overall headcount has dropped by 13,000 over the past year, including 9,000 layoffs under a restructuring announced in September 2025 targeting around $1.25 billion in annualized savings.

GSK laying off 1.2K+ in Belgium

GSK is laying off more than 1,200 employees in Belgium, part of a wave of biopharma job cuts in 2026 tracked by BioSpace. Other reductions include Novo Nordisk's workforce shrinking by about 13,000 people over the past year to 67,000 as it shifts money into R&D, Takeda cutting over 4,700 jobs, and Bristol Myers Squibb terminating 265 employees at its Princeton, New Jersey site between Dec. 17, 2026 and May 21, 2027. Layoffs tied to M&A include BioNTech letting go of around 800 people at CureVac.

Teclistamab versus lenalidomide-dexamethasone in high-risk smoldering multiple myeloma: a randomized phase 2 trial

Teclistamab produced higher rates of complete clinical responses than lenalidomide-dexamethasone in patients with high-risk smoldering multiple myeloma in the randomized phase 2 ImmunoPRISM trial. Longer follow-up is needed to determine whether it durably prevents progression to multiple myeloma.

Why it matters: First randomized comparison of teclistamab against lenalidomide-dexamethasone in high-risk smoldering myeloma.

Milvexian with Antiplatelet Therapy after Acute Coronary Syndrome Event.

Adding milvexian, an oral factor XIa inhibitor, to standard antiplatelet therapy within 7 days after an acute coronary syndrome did not reduce recurrent ischemic events. In 14,194 patients followed for a median of 12.2 months, the composite of cardiovascular death, myocardial infarction, or ischemic stroke occurred in 5.4% with milvexian versus 5.1% with placebo (hazard ratio, 1.05; 95% CI, 0.91 to 1.21). The trial was stopped for futility at a planned interim analysis; major bleeding rates were 0.3% in both groups.

Why it matters: A large phase 3 trial shows factor XIa inhibition with milvexian failed to improve outcomes after acute coronary syndrome.

Talquetamab-Daratumumab in Relapsed or Refractory Myeloma.

In a phase 3 trial of relapsed or refractory multiple myeloma after at least one prior line of therapy, talquetamab plus daratumumab (Tal-D) or with pomalidomide (Tal-DP) beat daratumumab-pomalidomide-dexamethasone (DPd) on progression-free survival: 24-month estimates of 81.3% and 77.6% versus 51.2%, with hazard ratios of 0.28 and 0.33. Complete response or better reached 71.1% and 69.0% versus 34.5%, and MRD-negative complete response 52.3% and 46.3% versus 15.9%. The results position GPRC5D-directed bispecific therapy earlier in the treatment sequence.

Why it matters: Phase 3 data show talquetamab regimens outperforming standard daratumumab-based therapy in relapsed myeloma after just one prior line.
