Lung neuroendocrine carcinomas with high DLL3 expression upregulate TPH1 to increase serotonin (5-HT), which induces immunosuppressive tumor-associated macrophages (TAMs) expressing HTR7, weakening CD8+ T cell function and driving resistance to tarlatamab.

Nomura et al. found that lung neuroendocrine cancer uses tumor-derived serotonin acting through the HTR7 pathway to program immunosuppressive macrophages, mediating resistance to tarlatamab and ICB. This neuroimmune mechanism holds in both SCLC and LCNEC, and points to HTR7 as a potentially druggable therapeutic vulnerability.
