Obinutuzumab Beats Tacrolimus in Phase 3 Membranous Nephropathy Trial
A phase 3 trial in the New England Journal of Medicine reports that obinutuzumab induced complete remission far more often than tacrolimus in primary membranous nephropathy. Among 142 adults, 26 of 71 patients on obinutuzumab reached complete remission by week 104, versus 4 of 70 on tacrolimus — 37% versus 6%, an adjusted difference of 31 percentage points (P<0.001). Grade 3 or higher adverse events were comparable, at 22% and 19%. The result positions the anti-CD20 antibody as an effective alternative to calcineurin inhibitors in a disease long short of proven options. Elsewhere, the FDA approved Scholar Rock's Isembyld as the first therapy targeting muscle loss in spinal muscular atrophy, and AstraZeneca's camizestrant missed its pivotal SERENA-4 endpoint in first-line ER-positive, HER2-negative advanced breast cancer.
Clinical readouts and safety
Obinutuzumab or Tacrolimus in Primary Membranous Nephropathy.
Obinutuzumab outperformed tacrolimus for inducing complete remission in primary membranous nephropathy in a phase 3 trial of 142 adults. At week 104, complete remission occurred in 26 of 71 patients given obinutuzumab versus 4 of 70 given tacrolimus (37% vs. 6%; adjusted difference, 31 percentage points; P<0.001). Grade 3 or higher adverse events were similar, at 22% and 19%. The finding positions obinutuzumab as an effective alternative to tacrolimus in this disease.
STAT+: Breast cancer pill from AstraZeneca misses mark in pivotal trial
AstraZeneca's oral drug camizestrant, sold as Etcamah, failed to beat standard treatment in the pivotal SERENA-4 trial as first-line therapy for ER-positive, HER2-negative advanced breast cancer. The combination did not extend the time patients went before their cancer progressed. The miss limits expansion beyond the accelerated U.S. approval for tumors with a drug-resistance mutation, though testing continues in early-stage disease.
Hydrogen-rich water combined with traditional Chinese medicine compound in the treatment of kidney stones: a randomized controlled prospective clinical trial
Adding hydrogen-rich water to Ye-Shi-Shi-Lin-Formula improved outcomes in kidney stone patients in a randomized controlled trial of 100 patients treated for 12 weeks. Total effective rates were 28% in the blank group, 76% with Chinese medicine alone, 80% with hydrogen-rich water plus Chinese medicine, and 32% with hydrogen-rich water alone. The combination showed superior effects on oxidative stress, blood uric acid, urinary stone-related metabolic factors, and renal tubular function.

STAT+: Scholar Rock wins FDA approval for first drug to target SMA muscle loss
The FDA has approved Scholar Rock's Isembyld, the first therapy to target muscle loss in spinal muscular atrophy, for adults and children aged 2 and older already receiving SMA therapies that target SMN2. In a late-stage trial, Isembyld combined with an SMN2-targeting drug improved young patients' motor skills over a year while placebo recipients declined, a statistically significant difference. Approval was granted on Friday.
NMPA Approves T-DXd Plus Pertuzumab as First-Line Treatment for HER2-Positive Metastatic Breast Cancer
On August 11, 2026, China's NMPA approved trastuzumab deruxtecan (T-DXd) combined with pertuzumab for first-line treatment of adults with unresectable or metastatic HER2-positive breast cancer. The approval rests on the global DESTINY-Breast09 trial, which showed efficacy exceeding standard treatment with a safety profile validated in clinical testing. Professor Liao Ning offers a comprehensive assessment of the regimen, citing durable survival and manageable safety.

GSR Identified as Common Metabolic and Immune Regulator of Ferroptosis and Cuproptosis in Tumors
Glutathione reductase (GSR) controls tumor cell susceptibility to both ferroptosis and cuproptosis, and shapes the immunogenicity of these two forms of regulated cell death. Genome-wide CRISPR screening, combined with genetic, pharmacological and in vivo experiments, showed that loss of GSR disrupts glutathione-dependent redox homeostasis and markedly increases sensitivity to ferroptosis and cuproptosis across multiple tumor models.
Early-life colonization with Clostridioides difficile remodels the developing gut.
Early-life colonization with Clostridioides difficile remodels the developing gut, driving proinflammatory and tissue repair responses, enriching injury-associated intestinal stem cell populations, and skewing differentiation toward secretory lineages in a neonatal mouse model. Transient colonization caused persistent changes into adulthood, but responses were toxin dependent: nontoxigenic strains and maternal vaccination with a C. difficile-targeted messenger RNA-lipid nanoparticle vaccine protected neonates. Human infant epithelial cells were toxin sensitive, and colonized infant biopsies showed altered intestinal stem cell behavior.
Effective tumor kinetics inferred from single routine H&E biopsies enable counterfactual virtual radiotherapy trials
Patient-specific proliferation and diffusion rates of a reaction-diffusion model can be inferred from a single routine H&E biopsy by fitting cell-nucleus point patterns, validated across eleven multicenter cohorts. The derived dispersion length and front velocity stratified progression-free and overall survival independent of routine covariates. A virtual glioblastoma trial allocating hypofractionation to faster-growing tumors and dose intensification to slower-growing ones added 96.7 days of restricted mean progression-free survival over standard of care (permutation p < 0.001).
Discovery and engineering of bispecific TCR-mimic antibodies targeting peptide-HLA complex
A fully human TCR-mimic antibody platform was built that combines pHLA immunogens, transgenic mice, single B-cell discovery and whole-proteome off-target screening (X-Scan) to generate antibodies recognizing peptide-HLA complexes. Binders against the Wilms Tumor 1 (WT1) antigen were engineered into bispecific antibodies that redirect T cells to kill tumor cells, outperforming competitors' TCRm antibodies. The work opens intracellular targets previously undruggable by antibodies.
Strain-resolved metabolomic chemotyping identifies tryptophan and γ-glutamyl peptides as MGO-AGEs breaking postbiotic compounds for carbonyl stress mitigation
A strain-resolved activity metabolomics framework screened 177 intracellular metabolite extracts from 113 bacterial strains for methylglyoxal advanced glycation end-product (MGO-AGEs) breaking activity, revealing functional differences among Limosilactobacillus fermentum chemotypes hidden at species level. The selected postbiotic extract LFEAN031, enriched in tryptophan, asparagine and three γ-glutamyl peptides, restored colon length shortening, raised colonic ZO-1, and lowered serum IL-6 and cortisol in MGO-stressed mice, with improved depressive- and anxiety-like behaviors and shifts toward protective tryptophan metabolites.
AI detects a distributed blood metabolomic Systemotype associated with early stage ovarian cancer
A machine-learning classifier detected early-stage ovarian cancer from untargeted metabolomic profiles with cross-validated sensitivity and specificity of 99.0% and 99.8% in serum (59 cancer, 32 controls) and 97.7% and 99.7% in plasma (63 cancer, 20 controls). Both cohorts shared 239 concordantly altered features across lipid, amino-acid, steroid, central-carbon, and redox metabolism, supporting a distributed metabolic 'Systemotype' detectable in blood.
Analysis of the electrogastrokinetic changes in a hyperthermia model of rat
Hyperthermia shifts gastric electrical activity toward spike-pattern waves in rats. Five rats kept at 42 ± 1 °C for 60 minutes showed more spike pattern waves on electrogastrography than five room-temperature controls (p < 0.01), with FFT analysis of full-length and five 15-minute segments. The finding links heat stress to altered autonomic activity and irregular gastric electrical activity.
Clinical and economic impact of bivalent respiratory syncytial virus prefusion F (RSVpreF) vaccination in older adults: a cost-effectiveness analysis for Chile
Bivalent RSVpreF vaccination of at-risk adults aged 60 years and older in Chile would prevent 14,956 hospitalizations, 50,419 emergency department visits and 124,849 physician office visits over a lifetime, while generating 7,079 QALYs and averting CLP76 billion (US$82 million) in direct medical costs. A Markov model put the maximum cost-effective vaccine price at CLP100,480 (US$108) from the healthcare perspective and CLP265,261 (US$285) from the societal perspective at a threshold of CLP15,977,348 (US$17,180) per QALY. This gives Chile's NITAG a price ceiling to act on its existing recommendation.
High levels of nitric oxide and peroxynitrite with protein nitration: a new therapeutic target for gliomas
Nitric oxide and peroxynitrite (ONOO-) drive tyrosine nitration of key proteins in the gap junction signaling pathway, disrupting intercellular communication linked to glioma progression. Nitroproteomics shows gap junction is one of the significantly enriched nitroprotein pathways in glioma, and nitration produces proteoforms with different functional states across glioma subtypes. The review proposes targeting gap junction communication activators, nitration inhibitors and specific nitroproteins, combined with chemotherapy or immune checkpoint inhibitors, as a therapeutic strategy.
