At #ESMO26, phase III PERSEE compared pembrolizumab + platinum-based chemotherapy versus pembrolizumab alone in first-line advanced #NSCLC with PD-L1 ≥50%.

HR 0.56 (0.43–0.74), p<0.001
➡️ Median OS: 50.2 vs 29.9 months
HR 0.77 (0.57–1.05)
➡️ ORR: 77.5% vs 50.3%
But the trade-off is substantial:
⚠️ Grade ≥3 TRAEs: 53.8% vs 19.3%
🎯 Why this matters: PD-L1 ≥50% may not be enough, by itself, to decide between pembrolizumab alone and chemo-immunotherapy.
For me, the real clinical question is now:
Which PD-L1–high patients need the deeper and faster disease control of chemotherapy upfront — and which can safely avoid the added toxicity?
That is where tumor burden, symptoms, pace of disease, comorbidities and better biomarkers will matter most.
