Epkinly Cuts Progression Risk 51% in First-Line DLBCL, Setting New Standard

AbbVie and Genmab's Epkinly added to R-CHOP cut the risk of disease progression or death by 51% in newly diagnosed diffuse large B-cell lymphoma, hitting the primary endpoint of the 900-patient Phase 3 EPCORE DLBCL-2 trial. The benefit far exceeded analyst expectations of 30-35%, was consistent across high-risk and low- to intermediate-risk disease, and surpasses the 27% and 25% reductions reported for Polivy and Monjuvi regimens, positioning Epkinly as a potential first-line standard of care. The day's other major clinical news included a 26.2-month median overall survival gain for daratumumab-based first-line therapy in transplant-ineligible myeloma in final MAIA analysis, phase 3 wins for obexelimab in IgG4-related disease and Hengrui's oral GLP-1 HRS-7535 over dapagliflozin, and a missed PFS endpoint for firmonertinib in EGFR exon 20-insertion NSCLC.

Hormonal therapy recommended as a first-line treatment for menopausal vasomotor symptoms

The American College of Physicians issued a clinical guideline in Annals of Internal Medicine recommending estrogen plus progestogen for females with a uterus, or estrogen alone for those without, as first-line treatment of menopausal vasomotor symptoms (hot flashes and night sweats), which affect up to 80% of females. For those with contraindications or intolerance to estrogen, ACP suggests an SNRI (desvenlafaxine or venlafaxine) as second-line treatment, then an SSRI, gabapentin or an NKra (elinzanetant or fezolinetant) as third-line. It urges clinicians to ask patients about vasomotor symptoms and weigh benefits, harms, cost and access in shared decisions.

A non-canonical function of glutathione to suppress ferroptosis via FSP1

Glutathione suppresses ferroptosis independently of GPX4 through a second, non-canonical mechanism: FSP1 uses GSH, rather than the NAD(P)H-FAD axis, to reduce ubiquinone and thereby inhibit ferroptosis. Targeting this FSP1-GSH axis alleviated ferroptosis-induced tissue injury, adding a GPX4-independent route by which GSH protects cells.

Mitochondrial proteome of Acanthamoeba delineates aerobic and anaerobic pathways dynamically regulated by oxygen.

The Acanthamoeba castellanii mitochondrial proteome has been mapped into AcMitoCarta, a 1,122-protein inventory built by immunoprecipitation, density gradient purification, mass spectrometry, protein correlation profiling and microscopy, with 381 proteins lacking obvious homologs in human and yeast mitochondria. Complexome analysis defines 20 macromolecular assemblies, and proteomic and transcriptomic profiling shows oxygen extensively rewires the organelle's bioenergetics, including induction of an anaerobic pyruvate:ferredoxin oxidoreductase-to-hydrogenase pathway under anoxia. Acanthamoeba is shown to produce H2 gas under anoxic conditions via a mitochondria-localized, oxygen-labile hydrogenase.

Chromatin accessibility trajectories reveal pre-cancerous epigenetic priming and an acid-driven immunotherapy-resistant gastric cancer subtype

A chromatin accessibility atlas of 650 gastric tissue samples from pre-cancer to advanced cancer reveals a cancer-like epigenetic priming state in metaplasia that licenses tumor initiation. Advanced cancers split into three epigenomic subtypes; the pseudo-gastric subtype resists immunotherapy through FOSL2-driven H+/K+ ATPase expression, which acidifies the microenvironment and suppresses CD8+ T cells, and the acid blocker vonoprazan reverses this phenotype. It identifies metaplasia as an epigenetically primed precursor and defines a mechanism-based resistant subtype.

Highly multiplexed mammalian metabolic engineering with a shotgun approach

Shotgun genetic engineering (SGE) delivers barcoded individual transcription units at high multiplicity of infection so each mammalian cell carries a unique pathway combination, letting a far larger design space be sampled in parallel than serial design-build-test cycles allow. After selection, sequencing of cells with the desired phenotype reveals which combinations of promoters, localization signals, enzymes, stoichiometry and copy number conferred the function. The method also generates large-scale synthetic datasets for training predictive machine learning models of pathway design.
