Deep dive

This readout belongs to the field of adjuvant chemotherapy for resected biliary tract cancer. The evidence boundary must be stated first: the three background references provided concern implementation science [1], integrated stress response biomarkers in sepsis-related ARDS [2], and vagus nerve stimulation [3], none of which intersects with adjuvant therapy for biliary tract cancer. They therefore cannot serve as the existing evidence base for this category, and the materials do not disclose prior pivotal adjuvant trial data for this indication (to be confirmed). Based on the live information alone, the design of ACTICCA-1 already frames the reference point: with capecitabine as the comparator, it tested whether gemcitabine + cisplatin could become the new standard in the adjuvant treatment of resected biliary tract cancer, and the live post is explicit on the verdict—these results do not establish gemcitabine + cisplatin as a new adjuvant standard.

Presented at ESMO 2026 in Madrid, ACTICCA-1 is a phase III trial analyzing 591 patients, comparing gemcitabine + cisplatin versus capecitabine as adjuvant therapy for resected biliary tract cancer. Median follow-up was 4.8 years. Median disease-free survival (SLE) was 25.7 vs 17.9 months, but the log-rank comparison did not reach statistical significance (p=0.097). A prespecified analysis—predefined in anticipation of a treatment effect varying over time—used restricted mean DFS over the first 3 years and showed an average gain of 2.6 months (p=0.011). For overall survival (SG), the medians were 58.1 vs 48.2 months, HR 0.82, p=0.103, likewise without statistically significant difference. On safety, toxicity was higher with gemcitabine + cisplatin, predominantly hematological.

Relative to existing knowledge, the advance of this readout lies in locating the benefit of gemcitabine + cisplatin in the adjuvant setting along the time axis: the prespecified restricted mean analysis suggests the effect is concentrated in the first 3 years, averaging 2.6 months (p=0.011), consistent with the pattern of numerically more favorable medians yet a non-significant log-rank (p=0.097). However, neither the conventional log-rank comparison nor overall survival crossed the significance threshold (p=0.097, p=0.103), and the benefit comes at the cost of higher hematological toxicity. Thus, despite the statistical support from the prespecified analysis, the live post's conclusion remains conservative: gemcitabine + cisplatin cannot be established as a new adjuvant standard on this basis.

As for limitations: the live post does not disclose the formal definition of the primary endpoint, stratification factors, specific toxicity grades and incidence rates, or subgroup data (to be confirmed); with 4.8 years of follow-up, the OS result of HR 0.82 with p=0.103 is insufficient to support a survival benefit conclusion; and the prespecified background of a time-varying treatment effect also cautions against a straightforward reading of the medians. All figures above are copied verbatim from the live post, and the complete results should await the formal report.
