The value of antibody-drug conjugates (ADCs) over chemotherapy in advanced HR+/HER2- breast cancer has phase 3 underpinnings: in TROPION-Breast01, among patients progressing on endocrine therapy after one to two prior lines of chemotherapy, the TROP2 ADC datopotamab deruxtecan versus investigator's choice chemotherapy (ICC) yielded a PFS hazard ratio by BICR of 0.63 (95% CI 0.52-0.76), with nausea (any grade 51.1%, grade >=3 1.4%) and stomatitis (any grade 50%, grade >=3 6.4%) the most common TRAEs [1]; more broadly, ADC development around targets such as HER2 has become a driving force reshaping breast cancer treatment [4]. The question APEX-Breast01 addresses is whether the same logic holds in a HER2-low population with no prior chemotherapy in the advanced setting.

LBA27 presented at this ESMO is the phase 3 APEX-Breast01: the biparatopic HER2 ADC TQB2102 (also referred to in on-site posts as Rolditamig DXd), developed by CTTQ, versus TPC chemotherapy. The study enrolled 543 patients with HER2-low recurrent/metastatic breast cancer and no prior chemotherapy in the advanced setting, randomized to TQB2102 7.5 mg/kg Q3W or TPC (capecitabine/paclitaxel/nab-paclitaxel); data cutoff was May 31, 2026. Median PFS was 16.4 vs 5.5 months (HR 0.29, p<0.0001), consistent in both HR+/HER2-low and all HER2-low; ORR was 68.8% vs 31.0%; OS showed a clear benefit trend (HR 0.44) but data are immature. Safety was manageable: grade >=3 neutropenia was most common, TRAE discontinuation was 1.5% in each arm, and ILD occurred in 2.2%, mostly grade 1-2. An NDA has been submitted in China.

Relative to existing knowledge, the readout advances on three fronts. In magnitude, the ADC-versus-chemotherapy PFS HR was 0.63 in TROPION-Breast01 [1] versus 0.29 here — though the control arms differ (ICC of eribulin/vinorelbine/capecitabine/gemcitabine [1] vs capecitabine/paclitaxel/nab-paclitaxel here), so this is only a cross-trial indirect comparison. In population, this study enrolled patients with no prior advanced chemotherapy, whereas TROPION-Breast01 required one to two prior lines [1], a clear shift toward earlier lines. In technical route, a biparatopic HER2 ADC achieved deep responses in HER2-low disease (ORR 68.8% vs 31.0%); on-site commentators summarized the magnitude as 'new ADC, same ~0.3 hazard ratio' and called for head-to-head trials.

The limitations should be stated plainly: OS data remain immature and long-term safety awaits follow-up; cross-trial HR comparisons are only directional; the available information comes from a phase 3 conducted in China with a China NDA submitted, and its generalizability and the drug's exact position within the ADC sequence await head-to-head trials and longer-term data.

Chia Tai Tianqing's TQB2102 Cuts Progression or Death Risk 71% Versus Chemotherapy in APEX-Breast01 in HER2-Low Advanced Breast Cancer

Chia Tai Tianqing presented APEX-Breast01 results for TQB2102 at ESMO 2026, showing it significantly improved median progression-free survival (mPFS) over chemotherapy in patients with HER2-low advanced breast cancer. TQB2102, a next-generation HER2 bispecific ADC, achieved an mPFS of 16.4 months versus 5.5 months with chemotherapy, a 71% reduction in the risk of disease progression or death, with treatment-related interstitial lung disease (ILD) occurring in 2.2% of patients.

Why it matters: APEX-Breast01 provides mPFS, progression or death risk, and ILD data for TQB2102 versus chemotherapy, allowing direct comparison of efficacy and safety.

CTTQ Reports Phase II First-Line Data for TQB2102 Plus Benmelstobart and Capecitabine in HER2-Positive Advanced Gastric Cancer at ESMO, Triplet ORR 84.2%

Chia Tai Tianqing (CTTQ) presented Phase II results at ESMO 2026 for TQB2102 combined with benmelstobart and capecitabine as first-line therapy for HER2-positive advanced gastric cancer, with the triplet regimen achieving an ORR of 84.2%. As of September 3, 2026, the TQB2102-II-04 study (NCT06767800) had enrolled 86 patients; the 6 mg/kg triplet arm showed an ORR of 95.8% (23/24), a cORR of 91.7% and a DCR of 100%, and this dose was selected as the recommended dose for the subsequent Phase III study.

Why it matters: The Phase II study compared doses by ORR, DCR and safety and selected 6 mg/kg as the recommended Phase III dose.

Sino Biopharmaceutical's TQB2102 Extends Median PFS to 16.4 Months in Phase 3 HER2-Low Breast Cancer Study

Sino Biopharmaceutical presented phase 3 results for TQB2102 in HER2-low breast cancer at ESMO 2026: median progression-free survival was 16.4 months versus 5.5 months in the chemotherapy arm. APEX-Breast01 was released as LBA27, with 543 patients randomized as of May 31, 2026; HR was 0.29 in both the HR+/HER2-low population and the overall HER2-low population, with 95% CIs of 0.22–0.39 and 0.22–0.38, respectively, and p<0.0001.

Why it matters: The conference abstract gives PFS effect size, ORR and ILD rate from a randomized phase 3 study, enabling comparison with chemotherapy.

ESMO 2026: TQB2102 Phase III APEX-Breast01 Shows PFS Improvement Over Chemotherapy in HER2-Low Breast Cancer

The Phase III APEX-Breast01 study of TQB2102, presented at ESMO 2026, showed significantly prolonged PFS versus investigator's choice chemotherapy in patients with HER2-low breast cancer. The study enrolled patients with HER2-low breast cancer at the recurrent or metastatic stage who had not previously received chemotherapy. TQB2102 is a HER2 bispecific epitope antibody-drug conjugate.
