At #ESMO26, long-term follow-up from phase III PEACE-1 shows that adding abiraterone + prednisone to standard of care in de novo mCSPC produces a sustained survival benefit nearly 8 years later.
📊 n=1,173 | median follow-up 7.7 years
➡️ OS HR 0.75 (0.65–0.87)
➡️ With ADT + docetaxel: median OS 6.2 vs 4.4 years
➡️ 8-year OS: 40.3% vs 27.1%
And in high-volume disease:
➡️ 8-year OS: 38% vs 20%
🎯 Why this matters: this is not just an early progression-delay signal. It is a long-term survival story.
For me, PEACE-1 reinforces an important principle in de novo metastatic #ProstateCancer:
when a patient is fit for intensification, delaying effective systemic therapy may mean losing survival that cannot always be recovered later.
The challenge now is less about proving that intensification works — and more about selecting the right triplet, for the right patient, while balancing toxicity, comorbidity and access.
