Common nutritional supplement may boost psoriasis treatment, small patient trial suggests

Topical therapy for psoriasis is a mature and crowded field: the calcipotriene/betamethasone dipropionate combination is well established, with its generic formally approved by the US FDA in December 2014 (ANDA200174) , and the FDA label confirms that calcipotriene ointment 0.005% is indicated for plaque psoriasis in adults . Industry is still advancing new topical options, such as the Phase III trial of MC2-01 cream sponsored by Hangzhou Zhongmei Huadong Pharmaceutical, comparing against CAL/BDP gel and vehicle (NCT07357831), with 539 planned subjects, primary endpoint of PGA success rate at 8 weeks, started December 30, 2025, primary completion August 6, 2026, status completed . In addition, a Phase 2 trial of acupuncture plus Chinese herbal cream versus acupuncture plus calcipotriol, sponsored by Mae Fah Luang University (NCT07410793) with 28 subjects, started February 9, 2026 and is still recruiting (needs verification).

This news comes from a peer-reviewed study by a Chinese team published October 27, 2026 in the Journal of Experimental Medicine (DOI: 10.1084/jem.20252558). Wei-Na Zhang of the State Key Laboratory of Biomedical Analysis in Beijing, together with the First Medical Center of Chinese PLA General Hospital and others, found that levels of Vanin metabolic enzymes are elevated in dendritic cells of patients with psoriasis and several other autoimmune diseases. Vanins convert the metabolite pantethine, a vitamin B5 derivative, into vitamin B5; elevated Vanin activity depletes pantethine, boosting cholesterol synthesis and driving dendritic cell hyperactivation. A drug inhibiting Vanins prevented mice from developing psoriasis, and replenishing pantethine reversed the process. The team then ran a small trial: 24 psoriasis patients all used topical calcipotriene/betamethasone ointment, and half also received pantethine tablets; over 12 weeks, the pantethine group showed improved symptoms, particularly among those with elevated blood cholesterol at baseline.

The significance lies in linking the common nutritional supplement pantethine (often used to lower blood cholesterol) to dendritic cell-mediated autoimmunity via the Vanin–pantethine–cholesterol axis, and validating in humans its feasibility as an adjunct on top of standard topical calcipotriene/betamethasone. The researchers themselves caution that this investigator-initiated trial is proof of concept for clinical feasibility rather than direct mechanistic validation, with only 24 participants. Relative to the existing landscape of approved combination topicals and advancing Phase III topical candidates , this study does not change standard of care but offers a low-cost oral adjunct direction, with a patient-stratification rationale tied to high cholesterol, a known psoriasis risk factor.

Not yet disclosed: dosing specifics, blinding or placebo design details, quantitative efficacy outcomes such as PASI changes, and safety data from the trial; plans for larger randomized controlled trials of pantethine as adjunctive psoriasis therapy are also undisclosed. Key follow-up points include whether the team advances this proof of concept into formal registered clinical studies, and whether the Vanin–pantethine–cholesterol axis is validated in other dendritic cell-mediated autoimmune diseases (needs verification).
