Deep dive

The population-level validation of the mRNA vaccine modality previously came from the prophylactic setting: BNT162b2 is a lipid nanoparticle-formulated, nucleoside-modified RNA vaccine encoding the full-length SARS-CoV-2 spike protein; in a randomized, placebo-controlled pivotal trial of 43,548 participants, after two doses (30 μg each, 21 days apart) there were 8 Covid-19 cases in the vaccine group versus 162 in the placebo group, with 95% efficacy [3]. This validation put the mRNA route through both efficacy and safety testing at scale in a large population; what therapeutic exploration of personalized neoantigen vaccines built on mRNA still lacked was evidence of translation into survival benefit.

At this ESMO meeting, the @myESMO live post pointed to a readout described as long-awaited: a personalised mRNA neoantigen vaccine reported positive survival outcomes; the specific trial name, tumor types involved, and survival figures were not disclosed in the post. The same post also flagged a second item: in his Editorial, John Haanen discusses how to make cellular therapies practical, scalable and sustainable. dr_dirican's takeaway post added that personalized mRNA vaccines may generate durable T-cell immunity, with signals even in low-TMB tumors and promising signals in pancreatic cancer and melanoma, while Phase III validation and broader access remain critical—framing cancer vaccines as whether, after 100 years of setbacks, we are finally getting closer.

Set against that background, the advance here shows on two dimensions, endpoint and scope: the modality's previous population-level validation came from a prophylactic vaccine with laboratory-confirmed Covid-19 prevention as the primary endpoint [3]; this time it is a personalized, therapeutic neoantigen vaccine delivering positive results expressed as survival benefit. The expert takeaways pointed the signals to pancreatic cancer and melanoma, and emphasized that durable T-cell immunity may be induced even in low-TMB tumors. For a field described as having endured 100 years of setbacks, this is exactly the kind of long-awaited result.

The limitations must also be stated: the live posts gave no specific numbers—details of the survival endpoints, sample size, and trial design remain to be confirmed; dr_dirican himself lists Phase III validation and broader access as still critical. As for the question Haanen raises in his Editorial—how to make cellular therapies practical, scalable and sustainable—it is a parallel thread of discussion alongside the vaccine readout, likewise without details in the post.
