Deep dive

Adjuvant pembrolizumab is already a standard after resection of stage IIB–IV melanoma. KEYNOTE-942 was a randomised phase 2b of individualised neoantigen mRNA (then mRNA-4157 / V940) plus pembrolizumab versus pembrolizumab alone; Lancet 2024 reported recurrence-free survival, not overall survival.[1] A 2026 JCO paper updates that trial at five years.[2] Those are phase-2 numbers, not this phase 3.

INTerpath-001 (NCT05933577) is the phase-3 follow-on, not the same trial: intismeran autogene plus pembrolizumab versus pembrolizumab alone. Floor LBA1 (Presidential I) reports n=1137, RFS hazard ratio 0.58, 2-year RFS 82.2% vs 70.0%; distant metastasis-free survival also met; OS immature. Floor copy says no new safety signals. Full confidence intervals, stage and BRAF strata wait on the slides.

What this adds to 942 is the same combination in 1137 patients, with both RFS and DMFS met. OS is immature; an RFS hazard ratio of 0.58 is not yet a rewrite of adjuvant melanoma care. Manufacturing turnaround and access for individualised mRNA are not in the floor posts. Keep the meeting numbers separate from the 942 papers.

#ESMO26 | INTerpath-001: My Personal Perspective Ahead of the Presidential Session This innovation deserves recognition: a groundbreaking result for personalized cancer vaccines. However, in patients with macroscopic stage III melanoma who are eligible for neoadjuvant IO, this trial does not establish the added value over contemporary perioperative strategies. Neoadjuvant treatment can produce substantial responses while allowing some patients to avoid adjuvant therapy. The trial design I would prioritize is: Neoadjuvant IO → surgery → standard adjuvant therapy ± a personalized vaccine in patients without a major pathological response. Here, “failure to achieve a major pathological response” is a more appropriate selection criterion than “any residual disease.” Patients with minimal residual viable tumor who still achieve a major pathological response should not automatically receive additional treatment. This approach would direct treatment intensification toward patients at higher risk of recurrence. The key biological uncertainty, however, is that IO-resistant residual disease is not necessarily vaccine-sensitive. Whether vaccination can overcome that resistance needs to be demonstrated. This criticism should remain specific to that population: INTerpath-001 retains distinct clinical value for patients with stage IIB/IIC disease and those who underwent surgery without neoadjuvant therapy.
