★★★Clinical trial

Nivolumab, pembrolizumab and durvalumab sit in this direction not as new drugs, but because perioperative IO trials wrote ctDNA into the stratification.

Other cases in this chapter
  • CheckMate 77TctDNA clearance 75% vs 46%; clearers ~70% pCR, non-clearers ~0%
  • PredictaClaimed sensitivity 1 in 10⁶
  • NSABP-B-59Tumor-informed WES predicted non-pCR and distant recurrence

So no one knows why Durvalumab worked and Pembrolizumab and Nivo failed in perioperative MIBC trials. @DrChoueiri @tompowles1 @HHammersMD @montypal @OncoAlert @myESMO #esmo26

Infographic to show the cross trial comparisons of 3 phase III perioperative IO trials in MIBC Only one is positive. Why ? I don't know. Looking forward towards full data of 2 negative trials on day 1 #esmo26 @myESMO

    #ESMO26 | POP-DURVA – Phase II

    引用@

#ESMO26 | POP-DURVA In stage I TNBC, the clinical and immunological effects of a single dose of durvalumab without chemo were evaluated (n=71). - pCR: 1.7% - Objective response: 11.6% - Tumor shrinkage: 41.3% - Conversion to an inflamed phenotype: 33% of initially immune-negative tumors. A single dose of immunotherapy induced substantial immune activation, but pathological response was very limited. The main value of this study lies in identifying potential biomarkers of immunotherapy sensitivity rather than demonstrating the efficacy of a chemo-free approach. However, results after only 15 days of treatment cannot establish the efficacy of longer-term monotherapy.
