Neoadjuvant therapy for locally advanced head and neck squamous cell carcinoma (LA HNSCC) is evolving from conventional chemotherapy toward immunotherapy-integrated regimens; 65-70% of patients present with stage III-IVA locally advanced disease at diagnosis. Current standards include surgery followed by adjuvant cisplatin-based chemoradiotherapy or definitive concurrent chemoradiotherapy, with cetuximab plus radiotherapy for HPV-negative patients unfit for platinum. Traditional induction regimens combine paclitaxel, platinum and 5-fluorouracil with radiotherapy, and early-phase trials are evaluating immune checkpoint inhibitors to improve organ preservation and tumor control.

Standard bulk partial EMT (pEMT) scores in head and neck squamous cell carcinoma mainly measure stroma: across three single-cell datasets, the malignant arm (laminin-332 and hemidesmosome genes) sat in malignant cells while the stromal arm (fibrillar collagens, vimentin) sat in fibroblasts. A classifier and 12-gene core recovering the malignant arm predicted long progression-free survival in 40 patients given platinum and cetuximab (AUC 0.78 and 0.82), but the malignant arm tracked the Basal subtype, whose score performed as well or better (0.93) and also in two xenograft panels.
