Synthesis

At ESMO 2026, Professor Huang Jun presented the phase 2 CRIT study of biomarker-guided neoadjuvant therapy in high-risk pMMR/MSS locally advanced rectal cancer. All 49 enrolled patients had treatment selected according to RAS/BRAF status, receiving either cetuximab beta or bevacizumab. The overall pathological complete response (pCR) rate was 51.1%, with 57.1% in the RAS/BRAF wild-type subgroup and 46.2% in the mutant subgroup. Of the 47 patients who proceeded to surgery, 24 achieved pCR, and the R0 resection rate was 100%. The findings suggest molecularly tailored neoadjuvant regimens can produce high response rates in a population typically resistant to immunotherapy. Open questions include whether the results hold up in larger trials and what longer-term outcomes such as recurrence and organ preservation will show.
