At #ESMO26, final results from RAMIRIS compared FOLFIRI + ramucirumab vs paclitaxel + ramucirumab as second-line therapy in taxane-pretreated advanced gastroesophageal adenocarcinoma.

➡️ Median OS: 8.7 vs 7.5 mo
HR 0.95, p=.33
➡️ ORR: 25.3% vs 22.6%
p=.29
➡️ Median PFS: 5.5 vs 3.7 mo
HR 0.70, p=.003
⚠️ Grade 3–5 AEs: 70% vs 68%
The co-primary endpoints were not met.
But the PFS signal is hard to ignore.
🎯 Why this matters: in taxane-pretreated GEA, FOLFIRI + ramucirumab may not replace paclitaxel + ramucirumab broadly, but it could still offer a meaningful alternative when prior taxane exposure, toxicity, neuropathy, or patient-specific factors make another taxane less attractive.
For me, this is exactly where personalized sequencing matters more than declaring a simple winner.
