Phase III PEACE-1: Long-Term Overall Survival With Abiraterone in mCSPC
At #ESMO26, Karim Fizazi presented long-term results from the Phase III PEACE-1 trial (2215O), with a median follow-up of 7.7 years.
Key findings:
▫️ Overall median OS: 6.1 vs 4.6 years with versus without abiraterone (HR 0.75; 95% CI 0.65–0.87; p=0.0001).
▫️ Triplet vs doublet therapy: Median OS was 6.2 vs 4.4 years (HR 0.68; 95% CI 0.56–0.82).
▫️ Eight-year OS: 40.3% with triplet therapy vs 27.1% with doublet therapy.
▫️ Metastatic volume: OS HRs were 0.67 in high-volume and 0.74 in low-volume disease, with no significant interaction (p=0.56).
Takeaway: Long-term PEACE-1 results demonstrated a sustained survival benefit with abiraterone, including when added to ADT plus docetaxel. The low-volume subgroup findings require cautious interpretation.
