#ESMO26 | PREDICT-HER – Phase II
HER2-PET is a promising approach to assessing target heterogeneity beyond a single biopsy. In HER2-low/zero patients, mPFS was 11.9 vs 4.8 months in PET-positive vs PET-negative groups; PET status remained independently associated with PFS after adjustment for pathological HER2 status.
However, the 100% NPV in PET-negative HER2-low/zero patients is based on only 10 patients (95% CI 69–100%). Absence of objective response does not necessarily mean absence of clinical benefit or primary resistance.
Another key point: a single positive lesion classifies the patient as PET-positive. The proportion of PET-negative disease burden may be more informative.
A valuable approach that could refine patient selection, but the evidence is not yet sufficient to withhold T-DXd based on PET negativity.

Possibly! In 89 patients receiving T-DXd, median PFS was 27.5 vs 13.6 months in HER2-PET-positive vs negative patients (HR 0.11). In HER2-low/zero patients, HER2-PET positivity had a 93.8% positive predictive value for response.
