In a new #OAOnTheGo video from #ESMO26, @scocmem discusses TROPHY Cohort 7, evaluating sacituzumab govitecan + enfortumab vedotin + PD-1 blockade in advanced #UrothelialCancer.
The activity is striking:
🔹 All 18 patients had tumor shrinkage
🔹 Responses were durable
🔹 No treatment-related deaths
🔹 But ~60% experienced grade 3 AEs
🔹 G-CSF support was mandatory
🎯 Why this matters: combining ADCs with different targets and payloads may deepen responses — but the therapeutic window becomes the real challenge.
For me, the key question is no longer simply:
“Can triplets work?”
It is:
“Can we identify the patients who need this degree of intensification, choose complementary mechanisms, and proactively manage toxicity well enough to preserve the benefit?”
With other frontline triplet strategies emerging — including biomarker-driven approaches such as FGFR3 inhibition in FORAGER — this balance between depth of response and treatment burden will be critical.
