In triple-positive mBC, can we do DB-09 induction and PATINA maintenance?

DB-09 made T-DXd + pertuzumab a 1L option. PATINA showed palbociclib + ET + HP maintenance improves PFS after induction (HR 0.75). DB-Guide puts them together.

How long to induce? In a DB-09 post hoc analysis, median time to maximal tumor reduction was ~11 mo (~16 cycles) overall and ~13 mo (~19 cycles) in HER2+/HR+ disease. A 6-cycle stop (~4 mo) lands well before most patients hit their best response.

DESTINY-Breast Guide (N=200, single-arm):
1️⃣ T-DXd + pertuzumab for 18-24 cycles (12m), or switch after 8 cycles with confirmed CR
2️⃣ Palbociclib + ET + HP maintenance
3️⃣ T-DXd retreatment at progression.

Primary endpoint: 24-mo PFS. QOL built in.
