Four important studies from #ESMO26 today:
1. Endometrial cancer (Lancet): Sacituzumab tirumotecan (TROP2-targeted ADC) improved median overall survival from 12.1 to 19 months after prior chemotherapy and immunotherapy.
2. dMMR endometrial cancer (Lancet): First-line pembrolizumab alone improved median PFS (time without progression) from 8.4 to 38.2 months vs chemotherapy alone. A promising chemo-free option, although chemo plus immunotherapy remains the current standard.
3. Osteosarcoma (ARTEMIS-011): Risvutatug rezetecan (B7-H3-targeted ADC) improved median PFS from 1.4 to 5.5 months vs chemotherapy in heavily pretreated patients, including adolescents. A rare positive phase 3 trial in this disease.
4. An important caution (MOIO): In patients whose advanced cancer was responding after 6 months of immunotherapy, extending treatment intervals to every 12 weeks was unexpectedly associated with higher observed mortality (19.9% vs 10.5%) compared with standard dosing. The phase 3 trial was stopped early.
Important advances, but also a reminder that less treatment is not always better.
