🔹 OS: 50% vs 54% (HR 1.17)
🔹 Distant metastases: 46% vs 29% (p<0.01)
🔹 43% spared non-beneficial oesophagectomy.
🎯 Verdict: Active surveillance offers organ preservation without a statistically significant OS difference, but higher distant metastases raise concern.
Could surveillance become a standard option for carefully selected patients?
