Pembrolizumab in Neuroendocrine Prostate Cancer — A Survival Breakthrough?
Pembro + carboplatin/etoposide vs chemotherapy alone (N=70):
• OS: 10.2 vs 7.1 months | HR 0.33
• rPFS: 5.1 vs 4.0 months | HR 0.66
• ORR: 28% vs 15%
• PD-L1 CPS ≥1: OS HR 0.16
A striking survival signal, particularly in PD-L1-positive disease.
But small numbers, post hoc subgroup analysis and no formal hypothesis testing.
Promising, not yet practice-changing.
