At #ESMO26, the first interim analysis of the phase II GERCOR-GONO DEWI study explores one of the most provocative organ-preservation strategies in localized dMMR/MSI-H gastroesophageal adenocarcinoma:
dostarlimab + watch-and-wait.

➡️ 1-year cCR: 62.5% (15/24)
➡️ No relapse or death among patients with 1-year cCR at a median follow-up of 16.2 months
➡️ Grade 3–4 dostarlimab-related AEs: 21%
➡️ 9 patients underwent surgery — all achieved R0 resection
🎯 Why this matters: this is not just an immunotherapy efficacy story. It is a potential shift in treatment philosophy.
For selected dMMR/MSI-H patients, the future may move from “treat, then operate” toward “treat, assess deeply, and preserve the organ when oncologically safe.”
The key challenge now is defining how confidently we can identify true complete responders and how durable that response remains over time.
