At #ESMO26, exploratory ctDNA analyses from phase III evERA Breast Cancer suggest that giredestrant + everolimus maintains PFS benefit across a range of biologically challenging post-CDK4/6i ER+/HER2− advanced #BreastCancer subgroups.
📊 Among 365 ctDNA-evaluable patients:
🔹 cTF ≥1%: PFS HR 0.52
🔹 cTF <1%: HR 0.61
🔹 ESR1 D538: HR 0.58
🔹 ESR1 Y537: HR 0.35
🔹 TP53-mutated: HR 0.65
🔹 PIK3CA-mutated: HR 0.55
🎯 Why this matters: the benefit appears to persist despite differences in ctDNA burden, ESR1 variants, and poor-prognostic molecular features.
For me, the key question now is whether these biomarkers are mainly telling us who is higher risk — or whether future analyses can identify who derives the greatest absolute benefit from this strategy.
