Talazoparib is a PARP inhibitor that acts by trapping PARP1 on damaged DNA, creating synthetic lethality with homologous recombination repair deficiency [3]. Its phase 3 evidence was first established in breast cancer: in the EMBRACA trial, patients with germline BRCA1/2-mutated advanced breast cancer were randomized to single-agent talazoparib or physician's-choice chemotherapy, with median PFS of 8.6 vs. 5.6 months (HR 0.54) [1]; however, the final OS analysis showed an HR of 0.848 (95% CI 0.670–1.073; P=0.17), with no significant survival difference [6]. In prostate cancer, the randomized, double-blind phase 3 TALAPRO-2 established the efficacy and safety of talazoparib + enzalutamide versus enzalutamide alone in first-line mCRPC [4] — the direct premise for understanding the current readout.
The ESMO26 meeting in Madrid focused on TALAPRO-3 (previously presented at ASCO 2026 and published in the NEJM, with an educational capsule breakdown by Joaquín Mateo of VHIO for the Guard Consortium). This phase 3 trial compared talazoparib + enzalutamide versus placebo + enzalutamide in patients with HRR-altered mHSPC, with 599 patients enrolled across both arms. The key numbers: 3-year rPFS of 77% vs. 56% (HR 0.48; 95% CI 0.36–0.65; p<0.001); OS at 3 years in the interim analysis of 78% vs. 72% (HR 0.77; 95% CI 0.56–1.04), with data still immature; grade ≥3 anemia occurred in 51% of patients on talazoparib. Two companion analyses were presented at the meeting: patient-reported outcomes showed no clinically meaningful deterioration in overall QoL versus enzalutamide alone, and the ctDNA analysis showed that week-5 ctDNA tumor fraction clearance was higher with the combination (82% vs. 68%) and was associated with longer rPFS.
Relative to prior knowledge, the advance here is moving the PARP inhibition plus AR blockade strategy validated in mCRPC by TALAPRO-2 [4] earlier into the hormone-sensitive setting and restricting it to the HRR-altered population; the central question raised at the meeting was whether the role of PARP inhibitors changes as a result. The absence of clinically meaningful QoL deterioration adds the benefit–risk support for this treatment intensification, while week-5 ctDNA clearance offers a dynamic efficacy signal earlier than imaging. The trade-off is equally clear: grade ≥3 anemia of 51% exceeds the 24% (17/71) grade 3–4 anemia reported in the early single-agent phase I trial [5], making dosing and anemia management under combination therapy a key issue for clinical implementation.
In terms of limitations, OS data remain immature with the confidence interval crossing 1, and whether the rPFS benefit translates into a survival benefit awaits longer follow-up; the rPFS result is at the 3-year landmark, and longer-term efficacy and tolerability, as well as the value of ctDNA clearance as a biomarker for clinical decision-making, remain to be confirmed in subsequent data.

Adding the PARP inhibitor talazoparib to enzalutamide improved prostate-specific antigen response in patients with high-volume metastatic hormone-sensitive prostate cancer in the randomized phase 2 ZZ-FIRST trial. Spatial transcriptomic analysis of paired biopsies showed that androgen receptor inhibition suppresses homologous recombination repair-related programs, and this suppression correlated with proliferation arrest rather than functional HRR deficiency.
Why it matters: Links AR inhibition to HRR program suppression, giving a mechanistic basis for combining PARP inhibition with androgen receptor inhibitors.

In TNBC cells, TTFields upregulate p21(WAF1)/CDKN1A independently of TP53 status. Combination with the PARP inhibitor talazoparib further enhances p21 expression, induces γH2AX-mediated DNA damage, and downregulates PDL-1. Both treatments trigger cell death via PARP cleavage while reducing anti-apoptotic proteins including p-STAT3, STAT3, and MCL-1.

Inhibiting ribosome biogenesis with BMH-21 resensitizes PARPi-resistant triple-negative breast cancer to olaparib. The drug combination was synergistic in 4 PARPi-resistant cell lines and in 3 BRCA wild-type TNBC cell lines with intrinsic PARPi resistance, but not in parental cells. In MDAMB231 cells, the combination increased γH2AX-positive cells up to 7.3-fold, SA-β-gal staining 5.6-fold and p21 3.6-fold, findings mirrored with CX-5461. Patient cohort data linked ribosomal pathway activity to MYC, proliferation, DNA damage and senescence.
