Weekly Insulin, CagriSema Head-to-Head Win and a Wave of FDA Approvals Define the Week

Eli Lilly's once-weekly basal insulin efsitora alfa (Onswik) won FDA approval for type 2 diabetes and a separate green light in China, challenging daily-injection insulin routines. Novo Nordisk's CagriSema beat tirzepatide head-to-head in the phase III REIMAGINE 5 trial while also meeting its endpoint in REDEFINE 9, setting up a direct challenge in obesity and diabetes. The FDA kept busy elsewhere, approving the first Alexander disease therapy (Ionis's Zanvastro), finerenone for type 1 diabetes-related chronic kidney disease, and a $3.95 million gene therapy for MPS IIIA, while an advisory panel backed GRAIL's Galleri cancer blood test. Late-stage readouts spanned IgA nephropathy, psoriasis, XLRP gene therapy and long-term CAR-T data, plus a $1.65 billion radiopharma merger and new FDA rules replacing animal testing.

Weekly Insulin Efsitora Wins US and China Approvals

On this pageThe FDA approved Eli Lilly's once-weekly basal insulin efsitora alfa (Onswik) for adults with type 2 diabetes, announced September 24, 2026, to improve glycaemic control alongside diet and exercise. Efsitora is a fusion protein linking a novel single-chain insulin variant to a human IgG2-Fc domain, extending half-life so a weekly subcutaneous injection replaces daily dosing. China approved the drug as well. Expressed in CHO cells, the 299-amino-acid Fc-fusion protein has 24 disulfide bonds and a 64 kDa molecular weight; in QWINT-1 and QWINT-2 it showed glucose lowering non-inferior to insulin glargine and insulin degludec.

Beacon edges ahead in XLRP gene therapy race with Phase III hit

Laruparetigene zovaparvovec (laru-zova) from Beacon Therapeutics is the first gene therapy to meet the primary endpoint in a Phase III X-linked retinitis pigmentosa trial, improving visual acuity on the low-luminescence visual acuity (LLVA) scale in the VISTA study. Responders gaining 15 letters or more included 31% of patients in the higher-dose cohort and 24.1% in the lower-dose cohort. The result raises the prospect of preventing or delaying blindness in XLRP patients.

Half of RRMM Patients Progression-Free Five Years After Single CARVYKTI Infusion in CARTITUDE-2

A single infusion of CARVYKTI (cilta-cel) kept 10 of 20 patients (50%) with relapsed/refractory multiple myeloma alive and progression-free at five years without maintenance therapy. The CARTITUDE-2 cohort A data, with a median follow-up of 60.7 months, cover patients who had received 1 to 3 prior lines of therapy. The results extend the durability evidence for cilta-cel into earlier-line RRMM.

Non-Animal Testing Rules and a Radiopharma Megadeal

On this pageThe FDA issued a direct final rule allowing validated, appropriate non-animal methods to supplement or replace animal testing for preclinical drug and biologic safety, replacing 'animal tests' with 'nonclinical tests.' HHS, in its second such announcement this year, paired the rule with $88 million in NIH funding for 10 infrastructure projects backing cell-based assays, organs-on-chip and computer models. In dealmaking, Telix Pharmaceuticals agreed to acquire Germany's ITM Isotope Technologies Munich for about $1.65 billion on a cash-free, debt-free basis, with up to $700 million more contingent on ITM's lead drug. ITM holders receive roughly $1.25 billion in Telix shares at $11.84 each, leaving them with about 23.7% of the combined company.

FDA Direct Final Rule Allows Non-Animal Methods to Replace Animal Safety Testing

The FDA has issued a direct final rule stating that non-animal methods can be used to assess the preclinical safety of drugs and biological products intended for humans. The change formally recognizes emerging approaches as able to supplement or replace animal testing in human drug safety evaluation. Non-animal methods may be used when they are adequately validated and appropriate for the product and regulatory question.

STAT News09.23

STAT+: HHS unveils new initiatives to reduce animal testing in drug development

For the second time this year, HHS announced strategies to shift drug development toward human-based research methods and reduce animal testing. The FDA issued a final rule allowing alternatives to animal testing when 'appropriate' for determining the safety of drugs and biologics, replacing terms like 'animal tests' with 'nonclinical tests.' The NIH will provide $88 million for 10 infrastructure projects encouraging methods such as cell-based assays, organs-on-a-chip, and computer models.
