Ponatinib-Blinatumomab or Imatinib-Chemotherapy for Adult Ph-Positive ALL.

In brief

Frontline ponatinib plus blinatumomab beat imatinib plus chemotherapy in adult Ph-positive ALL, with 24-month event-free survival of 86% versus 67% (P<0.001) at a median follow-up of 23.7 months. Complete hematologic response at end of induction was 94% versus 79%, and survival with no measurable residual disease was 68% versus 42%; cumulative relapse was 5% versus 13%. Only 19% of intervention patients proceeded to stem-cell transplantation after first complete response, supporting a chemotherapy-free frontline strategy.

Why it matters

Phase 3 data show a chemotherapy-free regimen outperforms standard imatinib-chemotherapy in adult Ph-positive ALL.
