Deep dive

Small-cell lung cancer (SCLC) accounts for about 15% of all lung cancers and is marked by an exceptionally high proliferative rate, a strong predilection for early metastasis and poor prognosis, with most patients already metastatic at diagnosis [6]; US SEER data show its share among all lung cancer histologies fell from 17.26% in 1986 to 12.95% in 2002 [5]. At the genomic level, functional inactivation of TP53 and RB1 is nearly universal [3][6]. In treatment, immunotherapy has established itself in first-line extensive-stage SCLC: the CASPIAN phase 3 trial randomized patients 1:1:1 at 209 sites across 23 countries, comparing durvalumab (± tremelimumab) plus platinum-etoposide against chemotherapy alone [4], and reviews note the recent introduction of immunotherapy reshaping the treatment landscape of this tumor type [6]. This is the class baseline against which any SCLC immunotherapy readout at this ESMO must be read.

As for this meeting specifically, BMS's official account previewed a rapid oral presentation tomorrow: pooled results from brain metastases subgroups of two Phase 2 studies (in SCLC and triple-negative breast cancer, TNBC respectively), with the post also carrying the #BioNTech_Group hashtag; the drug name, primary endpoints and any efficacy or safety figures were not disclosed and await the presentation. The same batch of live posts also includes an interview with Stephen V Liu highlighting the most anticipated lung cancer studies at ESMO2026, and Charles Rudin's forward-looking remark that 'when we ask the right questions, we will find the right answers' — these two are preview and color, leaving the brain metastases subgroup analysis as the only genuine new data point.

In terms of incremental advance, this readout narrows the lens to the specific population of patients with brain metastases. Background reviews already establish SCLC's strong predilection for early metastasis and that most patients are metastatic at diagnosis [6], whereas CASPIAN, judging from the abstract cited, did not address efficacy in a brain metastases subgroup [4]; presenting the brain metastases subgroups from two Phase 2 studies in SCLC and TNBC in a single session offers a concentrated test of efficacy signals in patients with central nervous system involvement, and reflects the move to dig into special populations on top of immunotherapy's established first-line position [4][6].

The limitations are equally clear: first, the data come from subgroup analyses of two Phase 2 studies, with evidence level and sample size below those of a confirmatory phase 3 trial [4]; second, the live posts provided no figures, so formal conclusions rest on the original conference presentation; third, the interview and quotes are forward-looking content rather than efficacy data, and readers should not draw any efficacy or investment conclusions from them.

@dr_yakupergun @SuyogCancer @myESMO @OncBrothers @KolPulseAI In my view, your opinions regarding DeLLphi-305 and IMforte are very sensible and very intelligent.Congrats! Unfortunately, #Tarlatamab is not for every patient, it cannot be administered in every setting, and it is not the most economical option. This must be taken into account.
