Deep dive

Within the immune checkpoint inhibitor class, the pivotal phase III evidence for pembrolizumab is well established: in previously untreated advanced NSCLC with PD-L1 expression on at least 50% of tumor cells and no EGFR/ALK alterations, single-agent pembrolizumab 200 mg every 3 weeks versus investigator's choice of platinum chemotherapy yielded median PFS of 10.3 vs 6.0 months [1]; in first-line metastatic nonsquamous NSCLC, a phase III trial enrolling 616 patients with OS and PFS as primary endpoints confirmed the value of adding chemotherapy [2]; in previously treated PD-L1-positive NSCLC, KEYNOTE-010 enrolled 1034 patients in a head-to-head comparison with docetaxel [3]; in advanced melanoma, KEYNOTE-006 enrolled 834 patients, with 6-month PFS rates of 47.3% (every 2 weeks) and 46.4% (every 3 weeks) versus 26.5% for ipilimumab [5]; and in first-line metastatic squamous NSCLC, the combination improved median OS to 15.9 vs 11.3 months, HR 0.64 [6]. The endpoints established by these trials were all systemic efficacy measures (PFS/OS), which is precisely the reference frame for understanding this lecture.

What was actually reported on site at ESMO26 (Spain) was not a new trial readout but Keynote lecture 1: speaker Wolfgang Wick (@wick_wolfgang) presented 'CNS disease as the next frontier in oncology care: Glioblastoma, metastases, and leptomeningeal disease' in three sections — a solid metastasis update, a leptomeningeal metastasis update, and glioblastoma. The live posts provided only the lecture structure and section titles; no specific trial names or any efficacy or safety figures were disclosed, and the underlying data remain to be confirmed.

Relative to the evidence framework represented by the background literature, the lecture's step forward lies in a shift of perspective: the pembrolizumab phase III program established standard therapy with extracranial/systemic efficacy as primary endpoints [1][2][3][5][6], whereas Wick placed three layers of CNS disease — parenchymal brain metastasis, leptomeningeal metastasis, and primary glioblastoma — side by side on the same agenda, positioning CNS disease as 'the next frontier' in oncology care per the lecture title itself. Putting metastatic and primary brain tumors into a single framework goes beyond the scope of any single anti-PD-1 trial, and suggests that, against the backdrop of established systemic therapy evidence, the management of CNS lesions is being singled out as a clinical issue in its own right.

The limitations must also be stated clearly: the live posts contain no figures or trial details, so this interpretation cannot confirm which studies the speaker actually cited; the background literature consists entirely of prior trials of pembrolizumab on extracranial efficacy and cannot be used to infer any efficacy or safety conclusions for the CNS populations discussed in the lecture — relevant content should await the official congress reports and abstracts. This article does not constitute investment advice.

Clinical trials rarely capture the full complexity of the patients we see in our practice. Great insights from @MarioBalsaMD on some of the challenges we face when evidence is limi
