For advanced non-small cell lung cancer (NSCLC) without actionable genomic alterations (AGA-negative), immune checkpoint inhibitor-based systemic therapy is anchored by published phase III evidence: among patients with PD-L1 expression on at least 50% of tumor cells and no sensitizing EGFR mutation or ALK translocation, pembrolizumab monotherapy versus investigator-choice platinum chemotherapy yielded median PFS of 10.3 vs 6.0 months [2]; in metastatic nonsquamous disease without sensitizing EGFR/ALK mutations, adding pembrolizumab to chemotherapy had already significantly improved response rates and prolonged PFS in a phase 2 trial, and was subsequently tested in a double-blind phase 3 trial of 616 patients with OS and PFS as primary endpoints [6]; in previously treated nonsquamous and squamous disease, nivolumab versus docetaxel improved median OS to 12.2 vs 9.4 months [3] and 9.2 vs 6.0 months [5], respectively. Meanwhile, the finding that EGFR mutations correlate with response to gefitinib [4] put driver-positive patients on a different treatment path, which explains why SARON restricted enrollment to AGA-negative patients: it asks whether consolidation radiotherapy can add value on top of immunotherapy-based systemic therapy.
Alongside several program items at ESMO26 in Madrid—Dr. Passaro co-chairing a symposium on the future of NSCLC and @Lung_Cancers teasing an interview with @LeXiuning—the genuinely new data readout was the randomized phase II SARON trial (presented by @fifimcdrmh). The study enrolled patients with synchronous oligometastatic, AGA-negative NSCLC (n=140) and added consolidation radiotherapy to first-line systemic therapy: median PFS was 13.9 vs 8.9 months, HR 0.64, p=0.03; 12-month local failure was 13.0% vs 39.5%, p<0.01; but median OS was 31.0 vs 28.0 months, HR 0.99, p=0.95; grade ≥3 AEs were 28.6% in both arms.
What this readout advances is the separation of local control from survival benefit: consolidation RT clearly improved disease control, cutting the 12-month local failure rate from 39.5% to 13.0%, and extended PFS with statistical significance, yet produced no OS separation at all (HR 0.99). The oncologist posting the summary accordingly emphasized that oligometastatic disease is not a single biological state, and that the value of local therapy may depend on patient selection, disease biology, timing, and whether controlling visible disease can meaningfully alter the systemic course. The identical 28.6% grade ≥3 AE rate in both arms also suggests the safety burden derives mainly from first-line systemic therapy rather than the added radiotherapy.
The limitations are equally clear: this is a phase II trial with n=140 and no OS difference; details such as the specific radiotherapy regimen, follow-up duration, and the definition of local failure were not disclosed in the conference posts, and whether the results generalize to AGA-positive or other oligometastatic settings remains to be confirmed.
