Pertuzumab is a humanized monoclonal antibody that inhibits HER2 receptor dimerization, with a mechanism complementary to that of trastuzumab [1]; in ErbB family biology, these receptors signal as homo- and heterodimers, and the ErbB2-ErbB3 heterodimer is among the most robust signaling complexes, making blockade of dimerization a rational strategy [5]. Its evidence base rests on pivotal randomized studies: the phase 3 CLEOPATRA trial in 808 patients with HER2-positive metastatic breast cancer showed that pertuzumab + trastuzumab + docetaxel significantly improved independently assessed progression-free survival versus the placebo combination in the first line [1]; in the final overall survival analysis with a median follow-up of 50 months, median overall survival was 56.5 versus 40.8 months [3]; in the adjuvant phase 3 APHINITY trial, recurrence occurred in 171 of 2400 patients (7.1%) in the pertuzumab group versus 210 of 2405 (8.7%) in the placebo group, hazard ratio 0.81 [4]; in the neoadjuvant setting, the four-arm phase 2 NeoSphere study compared various combinations [2].
Two pertuzumab-related items emerged at ESMO26. One is a post hoc observation from DB09: about 13% of patients received endocrine therapy as maintenance on top of T-DXd + pertuzumab, some possibly as a triple combination, but this endocrine add-on benefit has not been shown in prospective trials, and the expert noted it is currently difficult to recommend to patients. The other, and the main thread, is the DESTINY-Breast Guide design presented as abstract 1795TiP: in triple-positive (HER2+/HR+) metastatic breast cancer, it stitches together DB-09-style induction with PATINA-style maintenance—the on-site post notes that PATINA showed palbociclib + ET + HP maintenance after induction improves PFS (HR 0.75); a DB-09 post hoc analysis found the median time to maximal tumor reduction was ~11 mo (~16 cycles) overall and ~13 mo (~19 cycles) in HER2+/HR+ disease, suggesting that stopping at 6 cycles (~4 mo) comes well before most patients reach their best response. The single-arm study plans to enroll N=200: T-DXd + pertuzumab induction for 18-24 cycles (12 mo), or switching after 8 cycles with confirmed CR, followed by palbociclib + ET + HP maintenance, with T-DXd retreatment at progression; the primary endpoint is 24-month PFS, QOL is built in, and it is recruiting in the US, Canada, Europe, China, Korea, and Australia.
In terms of the step forward, pertuzumab's first-line evidence was previously built on a docetaxel backbone [1][3], and the on-site post states that DB-09 has made T-DXd + pertuzumab a first-line option; DB-Guide goes further by asking, in triple-positive disease, how long ADC + dual-antibody induction should run and when to switch to CDK4/6 inhibitor + endocrine + dual-antibody maintenance—the 18-24 cycle induction setting matches the time to maximal tumor reduction above, and T-DXd retreatment at progression rounds out the sequencing.
Limitations: DB-Guide is single-arm with 200 patients and no randomized comparator, and remains a trial-in-progress design (TiP) with the 24-month PFS primary endpoint result yet to come; the endocrine maintenance benefit suggested in the ~13% of DB09 patients is a post hoc observation lacking prospective evidence, and is not yet sufficient to support routine clinical recommendation.
