Clinicopathologic Evaluation of Amyloid Clearance in Alzheimer Disease.

Extensive amyloid clearance after aducanumab tracked with less downstream Alzheimer pathology in a clinicopathologic case report of a man in his 50s with mild cognitive impairment carrying the p.R47H TREM2 variant, who received 30 doses (cumulative 280 mg/kg) over 4.5 years. At autopsy four years after his final dose, brain regions with low residual amyloid, preferentially at gyral crests, showed less tau and slower longitudinal atrophy on MRI (β = -0.50; P < .001), while regions of high amyloid in sulcal depths had tau levels similar to those of untreated controls.
