International consensus sets updated framework for CAR T-cell therapy in severe autoimmune diseases
A group of 31 international experts has published updated consensus recommendations in The Lancet Rheumatology on using CAR T-cell therapy for severe, treatment-refractory autoimmune diseases. The 2026 recommendations, developed by EBMT, ISCT and JACIE with input from EULAR and ECTRIMS, update the first best-practice guidance from 2024 and cover patient selection, treatment in experienced FACT/JACIE-accredited centers, and reporting of patient data to the EBMT Registry. The panel calls for treatment within clinical trials where possible and lists larger controlled trials, biomarkers and novel CAR targets as research priorities.
Why it matters: New international guidance shapes how clinicians select and manage CAR T-cell patients in autoimmune disease, a fast-growing off-cancer use.
